Patients are required to get a physician's referral prior to scheduling an appointment. Patients should also gather any diagnostic or treatment records for pain for physician review This is to prevent unnecessary duplication of testing or treatment.
Insurance coverage
The pain treatment experts at Advocate Sherman Hospital are not employees of Advocate Health Care or Advocate Sherman but have been granted privileges to practice at the hospital. It's recommended to verify with your health insurance provider that both the hospital and physician group are in your plan before making an appointment.
Location
The pain treatment experts are on the first floor of Advocate Sherman Hospital. Hours are from 9 a.m. to 5 p.m. Monday through Friday (closed for lunch noon-1 p.m.).
What to expect
At your first appointment, you'll receive a thorough evaluation to determine the cause(s) of your pain. The pain treatment experts will review your medical records, obtain a complete history and physical and then design a treatment plan for your specific needs.

- Patient registration and pre-treatment assessment forms, which may include:
- All previous and relevant imaging studies with imaging report
- Previous medical records
- List of questions you may want answered
- Insurance card
- Driver’s license
