The following information may be helpful to you and your loved ones when it's time to deliver your baby at Advocate Christ Medical Center.

Packing checklist

We want you to be as comfortable as possible during your stay, so bring your robe, slippers and a gown or pajamas. If you do not want to bring your own toiletries, the medical center can provide a small kit that includes a toothbrush, toothpaste, soap, deodorant, body lotion, baby oil and tissues. For more information on packing for your hospital stay, you can use this checklist.

Patient admitting process & guidelines

Patients will be admitted directly to the labor and delivery unit. During the admission process, patients must sign a health care consent form and provide their demographic information. Patients can choose to submit this information ahead of time by using our online OB pre-registration form. The form covers consent to treat, responsibility for payment, assignment of benefits, release of medical information, independent physician services and declares that the medical center is not responsible for patients' valuables. Also a COB (coordination of benefits) form must be signed that asks whether a patient has other insurance coverage.

If you have not already done so, check with your place of employment or your insurance company to confirm the type of coverage that your insurance policy provides or to determine any unique requirements in your policy such as pre-admission certification or required notifications. All noncovered medical center charges are payable when you are discharged.

Bring the following information with you:

  • Insurance card, referral or pre-certification form or number.
  • Any written orders or paperwork the physician may have given you.
  • Name and phone number of your pediatrician.
